A 74-year-old female, presenting with complaints of abdominal pain and unexplained weight loss, was initially evaluated with a CT scan, scope and biopsy and was found to have signet ring cell carcinoma of the stomach. The patient was referred for staging with FAPI PET-CT. Apart from the fibroblast activation protein (FAP) expressing gastric lesion, imaging also revealed the presence of a bland thrombus in the splenic and portal veins. Signet ring cell carcinomas induce a state of hypercoagulability owing to mucin production, leading to multiple vascular thrombotic phenomena that herald a poor prognosis for the patient. And hence, early identification of Trousseau’s syndrome as a clinic-radiological entity is critical for patient management.
